We utilize an Electronic Health Record (EHR) system called Cerbo to maintain and manage your PHI. Cerbo is designed to ensure the confidentiality and security of your PHI and is compliant with HIPAA regulations.

  • Right to Rectification: You can request the correction or updating of inaccurate or incomplete PHI.

  • Right to Data Portability: Upon request, we can provide you with a copy of your PHI in a structured, commonly used, and machine-readable format.

Complaints

If you believe your privacy rights have been violated, you have the right to file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
